New Clients are required to complete the following 2 forms.

2.  Cancellation Policy

You may print and complete your forms ahead of time. Your signed forms will be reviewed and collected at the time of your first session.
Your completed forms, as well as everything that is discussed at the time of your session is confidential.
If you are unable to print your forms ahead of time, they will be provided and expected to be completed at the beginning of your first session.
